Consistent with our century-old tradition of high-quality care, the mission of the Visiting Nurses of the Lower Valley, Inc. (VNLV) is to provide comprehensive, personalized services that foster health, independence, dignity and comfort through home-based healing, education, and health-oriented community outreach initiatives. The Visiting Nurses of the Lower Valley, Inc. strives to enhance the health of the communities we serve through the application of public health measures and the provision of selective health services for purposes of preventing disease and disability, maintaining and restoring health, and minimizing the effects of illness and disability.
Visiting Nurses of the Lower Valley, Inc. will be recognized as the quality and value leader in community home care. The objective of Visiting Nurses of the Lower Valley, Inc. is to provide high quality nursing care, rehabilitative services, medical social work and home health aide services for people in their homes at a reasonable cost, while maintaining the highest standards of excellence. Each individual and family has unique responses to illness. The primary care nurse is the case manager responsible for the care plan best suited to these responses. This plan is jointly drawn up by the patient’s physician along with health professionals and the patient / family. The nurse utilizes the nursing process to assess, plan, implement and evaluate the care provided to individuals and families. Services are based on an inter-disciplinary approach that includes skilled nursing, physical therapy, speech therapy, occupational therapy, medical social work, home health aides, and nutrition consultation. There is a strong commitment to program evaluation, quality assurance, and maintenance of standards set for home health care by federal and state regulatory agencies.
